This facility is terrible. There are flies during the summer everywhere. The staff could care less about the patients. Food is not real food - smells bad and looks like dog food. Staff is more interested in talking shop with other staff members rather than care for their patients. They never answer the phone. DO NOT TAKE YOUR LOVED ONE THERE. read more

I haven't been here, but I work for a local hospital, this facility had one of their elderly, wheel chair bound, hard of hearing patients dropped off at their doctor's appointment with NO CNA/CAREGIVER, no assistance whatsoever! They didn't hear their name called, no one told us the patient was here, nothing! So if you do go here make sure you have someone advocate for you at the very least! read more

Inclined towards 1 star, but they get 2 stars only because I know there are genuinely worse facilities in the Legacy nursing home system- avoid this entire system if you can. My mom was admitted to their rehab unit- what a nightmare. Initially she was put in a shared room that was insanely loud (inconsiderate roommate) and suffocatingly hot- this could have been fatal to my mom who has breathing problems. She was given NO MEDS for a day and a half (Parma UH was largely to blame for this gross error) and an inappropriate diet. Was not given dinner at all on day 1- I had to go buy food at the store at 8pm! We asked repeatedly- but the kitchen is UNRESPONSIVE! Even after moving her to a single room on day 2, the food situation was never right- there are basically no alternatives to the main menu (my mom has a Rx highly restrictive diet) outside of a pathetic excuse for a PBJ sandwich-which she had for 4 dinners out of 10. We brought in food to supplement or replace the food served- which all looked pretty bad and is very skimpy. There is highly uneven personal care, and uneven responses to calls for assistance. It depends who is working the shift. Facility lacks good climate control. Rooms all tend to be suffocatingly hot- they all have wall unit AC's that blow colder air up against the ceiling in an attempt to regulate temps- this isn't a good system but the infrastructure for better air temps and circulation just isn't there. Physical and Occupational Therapy gave misleading, partial-picture reports- yes my mom could walk if you help her up, but she cannot get up and down at all! OT admitted that they "didn't work on that!" Their glowing reports got her cut off of Medicare. She was in absolutely no shape to be sent home- but we were still cut off. A day and a half before leaving she developed pneumonia. They gave her an oral med but it got worse- to their dismay I called 911 and she went back into the ER and was re-admitted. It took heavy doses of IV antibiotics to kick it. They offered to find a way to keep and treat her- umm, no thanks! I pity any elderly or disabled person without a family advocate- if my sister and I had not provided food, checked on her meds and been around to do the small things that make a difference to a bed bound person, I don't know what might have happened. Worst facility I've ever encountered. No matter what time you call they NEVER answer the phone. They need to be investigated due to lack of care and responsiveness. My Uncle, who is low maintenance, would press the call button to no avail. I went to what was supposed to be a care meeting ( it ended up being an "I DON'T CARE MEETING "). No one came except an office coordinator..... no nurse, no therapist, apparently they were too busy for a scheduled meeting. Laundry never came back, don't know what happened to his visibly marked clothing. They were supposed to administer a flu shot, he was there over 2 weeks and never received the shot. No matter who you talk to the right hand doesn't know what the left hand is doing. Beyond frustrating. The ineptness is stunning. read more

My Dad is there. Since getting there he has been on a mush diet, told them he is starving, I tried to call for a week and can NEVER get anyone on his floor. Finally got a supervisor on Friday at 730 pm and was told speech therapy evaluates 2-3 times a week but this week saw him once. Now that it is a weekend no one will see him until next week and he is hungry. When trying to reach ANYONE medical on his floor the phone will go unanswered and the longest I let it ring was 10 minutes. The excuse is we are dispensing MEDS. They must dispense meds at 10, 11 am and 1pm 2pm 3pm 7pm all day with no back up to answer their phones. Do not put your loved ones here. read more

After being in this facility for about two weeks, my mother has been readmitted to the hospital. She was being kicked out of the facility due to good physical therapy evaluations, however they admitted that they never evaluated her with an occupational therapist. She could not get up and down nor dress herself or do any of the basic daily functions. Over the course of the last few days she developed pneumonia and then they were very willing to keep her saying that they could treat her despite her having to foot the bill since Medicare would not cover it anymore. Our family chose to have her admitted to the hospital for proper care. Additionally, the cafeteria management is unresponsive and poor at best. They were listing an alternative diet better suited to her dietary needs however they would never follow through with requests for substitutions. The first night she stayed there they did not deliver her dinner. After multiple notifications that she did not receive food, they still never brought her any food. She was living off of soup and sandwiches. Ourfamily had to supplement her diet in order that she had enough to eat. We will not be returning her to this facility upon release. read more

If I could give this facility 0 stars, I would. Their complete and total neglect of my father after only a week in the facility caused him to spend a week at Parma General to take care of his dehydration and the UTI he got while he was there. After two days of hydration, the doctor at Parma said he was still "dry as a bone." His veins collapsed to the point that they had to use an ultrasound to find a vein. Instead of keeping my dad in his room and his bed, they wheeled him from room to room in a leather reclining chair so they could keep an eye on him so that he didn't fall out of bed (again.) But because no one actually took responsibility for him, he was not getting the food or adequate liquids he needed. He also started twitching, which was one of the signs of his dehydration level. NEVER in a million years would I have thought a facility with a supposed good reputation for therapy could do so much damage in such a short time. If you're looking for a good place for your loved one, look elsewhere. read more

If I could give zero stars then I would. This place is horrible horrible horrible. They treated me as if I were stupid. As a nurse practitioner with severe health issue, I was very knowledgeable and they really didn't like that. The things they tried to get away with is unbelievable. The things they got away with is unbelievable. They repeatedly had agency nurses in and did not explain to these agency nurses what all they were required to do if they were RN's. They treated me so disrespectfully and utterly without any thought to what they were doing to me. Making sure medication that I NEEDED, was taken away because they didn't like that I did need it(and it wasn't a narcotic). The food is so disgusting. They honestly do not offer snacks and if you want a decent meal forget it you won't get that here. You want something extra forget if you won't get it here. It's sad how they treat people who know what's going on. I left there and now I'm in a wonderful wonderful wonderful place. Where I'm treated with great respect and they aren't threatened with my knowledge of things. Sad that people are treated so awful here and I go someplace what and I'm treated so good. This place is the worst please do not put loved ones here. my brother had a bad stroke cannot talk can't eat and has part of skull removed for swelling and they told insurance he's uncooperativr in therapy so insurance refuses to pay. now have to pay out of pocket while he's lying there with no therapy all day not progressing in recover physical therapist is horrible and has no empathy never read his chart see what happened to him. this place needs to be investigated by the state licensing department. read more

My father recently stayed at Pleasant Lake Villa for IV therapy on the skilled unit where he had a beautiful, large size private suite. This was our first time having to find a SNF for him so it was a hard decision on the right place to put him. We were more than happy with our choice as he had the very best care here! He loved the food, loved ALL the staff and was treated like family. His room was very comfortable with plenty of room and privacy and made him feel at home. Our family was kept up to date on all of his care and his admission and discharge was very smooth. I highly recommend Pleasant Lake Villa to anyone looking for SNF for their loved ones! read more

This place is HORRIBLE! My brother was transferred to this place for wound treatment for his amputated toes. A few days later they took the Vac that's draining the wound off of his foot because according to them, it was broken. The next day, it's not broken, we used the wrong tubing. They left it on the bed and STILL have not put it back on for 2 days so far. We asked the nurse why is the pump not on yet and she responded that the doctor told them to leave it off until the his appointment. We called the doctor the next morning and he NEVER said that!! We're looking for a more responsible rehabilitation center to have him moved. This place will be the cause of my brother losing his foot. Total NEGLIGENCE. read more

I had redo my review about this place! Awful dont a need a one star at all they need to fire every one and get new people on down to administrator , this place is awful! My husband was there and they treated him so wrong, left him on bed pan until next day, stna nurse curse at my husband and when the nurse returned in room to finish her job, she go bring other nurse in and go say to my husband , if you had a problem? You could come and talk to me, WHO WANT TALK TO YOU AFTER YOU CURSE THEM OUT? Do you know they didnt fire her? They just put her somewhere else in nursing home, they clean him up all rough and moving him rough! His personal items came up missing, and his clothes, they will wash clothes and never bring his clothes back , they try give you other people clothes, the weekend is rough, short of staff stna and operator and they will NOT NEVER ANSWER YOUR CALLING LIght,. Even operator dont be there on weekend and when the week days come they be so snobby on the phone! I HATE THIS PLACE! I went facebook , tictok, instam gram and say never sent your love one to pleasant lake villa! They treat you good in the beginning but after that? Its like you being in jail! this place is horrible - don't even consider going here. when my mother was dropped off, she was put in a room, not 1 nurse or aide come to introduce them selves. my 85 year old mother was transferred there from metro for rehab - what a joke! they hardly ever got her out of bed.. she would push her call button and would take an hour or more for a nurse to come. she had asked for help with her tea, they told her to do herself. they told her to cut her own food.. so much for a rehab facility. they were to get her up and around instead put her in a diaper and left her in bed! the night crew is absolutely useless - no personality what so ever...never a conversation, nothing. Michaela one of the nurses, was clueless....waste of time even talking to her... stay far away from this place, i would not trust them to take care of anyone! Mom has been here for a year now. Previously she was in a facility in North Royalton. The staff here sometimes seem overwhelmed, but very capable. Here's the thing - Mom HATES the food. To the point where she will not eat it. I've checked her tray - to see if she's being, well, her....I'll eat anything, and I wouldn't touch it. It looks terrible. Why? Hamburger helper would be more appealing. Where are you getting this stuff? Mom calls it prison food (no drama here). So now I have to bring her dinner. Of course we still have to pay. Yes she's safe. Yes she's being cared for, and those are my priorities. She's being given Boost with meals, in case she won't eat it. The Boost contains milk, and she's lactose intolerant. She spends most days in the bathroom. They've been told several times she's lactose intolerant; it doesn't seem to filter to her meals. I do wonder if she's the only person there with dietary restrictions. read more

I'm updating my review because I'm back In after injuries from a fall. I'd still only give them 4 stars. The staff is still terrific and the food is marginally better but they changed the snack policy.. the only way you can get snacks is by calling the kitchen. They rarely answer the phone after dinner so if you don't call before that you don't get a snack all night. Unless they have ice cream or graham crackers in your floors dining area. I have a mom who has been at pleasant lake villa for 5 years. I have noticed the building and resident areas are kept exceptionally clean. The staff seems to be knowledgeable and overworked. If I could give zero stars, I would. My mother passed away there the night of February 25, 2021. Her funeral was March 4, 2021. We received a bill dated February 25th. On March 4th, day of funeral, we get a certified letter from this place saying they are going to send it to collections. So no time to grieve, plan funeral home, funeral, contact relatives according to this place, pay first. She wasn't even buried and this was sent. Real classy. read more

I loved doing the word puzzles in the Daily Chronicle, it's so nice that they do that here. The kitchen grounded up my meat for my meals so I could chew it better. This place they say has 5 stars for Parma's nursing home,I dont know where they got there info from! Its terrible after my dads stroke he was admitted at this facility and stayed due to his dementia getting worse .Everytime we called they never connected us to him or the nurses,we weren't able to visit due to covid and yet he ended up getting covid from a nurse there and died within 2 weeks after. They have lost his belongings several times.And now they won't return my calls or give me his personal belongings.I am beyond angry and will report to the BBB read more

HORRIBLE. Please do yourself a favor and don't take yourself or loved ones there. They lack compassion and are so cold hearted. My grandma had to go back to the hospital after her day and a half visit because the lack of care. Once we said to the nurse we will keep you updated on how she is the male standing near her said "as if we care" while we were walking away. This place is horrible. They treat people less than human. I went there after I developed an infection after open heart surgery. They had to give me an antibiotic once every 24 hrs. They completely missed the 1st day. I called the nurse many times through the day asking, and everybody ignored me. The nurses were cold, uncaring, and had a complete disregard for anything I said. I would like to start off by saying that my mother was a patient at Parma Hospital and Pleasant Lake Villa was highly recommended by the social worker and she couldn't say enough about this place. I will be honest that we did choose Mt. Alverna and I will explain why. We arrived at the facility for our tour, it was clean, the waiting area was very nice, as well as the receptionist Kim. We toured the facility with Kalee who was not very knowledgeable when we asked questions. She was short with her answers, was rude, and quite honestly acted very immature. She never smiled and just looked irritated that we were even there. She reminded me of a snotty spoiled child. Also, I am not in the position to body sham anyone as I myself is a plus size women but I know how to dress properly. This girl's attire was so inappropriate and I am appalled that her boss lets her dress like that at work. Tight, short dress with the back of your thighs and backside hanging out is NOT appropriate in the workplace let alone someone her size. This is girl needs to learn how to DRESS PROFESSIONALLY AND APPROPRIATELY FOR HER SIZE. She obviously is comfortable in her own skin which is fine but leave that for outside of work not where patients are there to get better. Everyone else we saw during our tour was dressed appropriately. This facility brags about being 5 star and they possibly could be because the facility is updated, clean, and very nice but first impressions are everything and you can only make one first impression as the saying goes. It may not be fair to give this facility such a poor rating seeing that I did not bring my mother there but Kalee is a reflection of that facility and if the admissions director (we asked the receptionist what she did there because she did not tell us when she introduced herself) treated us this way how do you think the other staff who will actually be taking care of my mother treat her? Are they as rude, short, unknowledgeable, easily irritated, and immature as she is? We didn't want to take the chance so that's how we ended up at Mt. Alverna. read more
