My Pops was on respite care for a week to give my sister a break. It was clean, nice patio outdoor space, and staff were pleasant. This review is updated eight years later because a loved one is here. I'm looking into moving her because this place is deplorable. The staff yells at her, she is a dying woman with dementia. I shouldn't have to tell them repeatedly that she needs help eating, yet, I do. They serve cold, unhealthy meals. Yesterday, I witnessed a CNA fake feeding a woman who hasn't been awake for four days of my loved one being there. The CNA just tapped the spoon on a bowl and was on her phone pretending to feed the resident. The food is horrible, served cold and there is no one to help feed feeble patients. All meds PRN ... ridiculous!!! The staff is inattentive except for those who are from a temp agency. No one knows how to get a television to connect to THEIR internet. I've asked twice a day for four days in a row for a phone call from Dr. SERANO and he's still not called me, or contacted me while I'm there two to three times a day. My loved one is on Comfort Care with no one knowing or seeming to understand that, although they were told by the hospital prior to the transfer. They don't appear to be properly medicating my loved one at all. Also no one is monitoring visitors, very unsafe. Shameful treatment of patients. I'll be trying to find another place for my loved one. read more

This facility came highly recommended by Hospital and was on a list of preferred providers. I selected this facility based on their rating and the wonderful things the hospital staff said about this place. Had a great 4 week stay here for Rehabilitation and wound care. Clean comfortable environment and had my own TV and patio outside my room. Staff was pleasant and took good care of me. The hospital was right, this is a great place and would recommend it. read more

I visited without an appointment so I could get an accurate feel for the place. I was greeted by Michael, the administrator who was happy and available to give me an overview. We took a look at the dining room and a couple open empty rooms. The rooms seemed quite small and a bit dark, right off the top each room has one tiny TV per 2 or 3 beds. Wow... This place is the most unprofessional medical anything I and my family have ever experienced. I say this with CONSTANT contact for the last 30+ years with medical professionals re; disabled family member. That isn't even getting to the facility itself which is old run down. Not even a phone in the rooms. ONE CORDLESS PHONE for the patients in the entire facilty. Ok but they dont answer it in the evening. A couple of CNAs have been rude and confrontational. Nurses, mostly ok with only 1 exception. Parking? Play the lotto, you'll have better odds. Management is a joke Inattentive is probably being waaay too nice. read more

Pine Ridge Care Center was a place recommended by the hospital for rehab and recovery. This place provided great care and exceptional service. The staff were great and they all showed care and compassion. During the stay of my elderly family member I got spend some time with some of the nursing staff and others from different departments that were involved in the care, I appreciate what they did for us and I highly recommend Pine Ridge Care Center. read more

great faciiity with good people that provide excellent care. I was at Pine Ridge for nearly 2 months after a bad fall at home. I had an 8 day hospital stay before being admitted to Pike Ridge. I was in room 32 and very thankful for all the care that was provided to me and the Rehab staff that got me back on my feet much stronger. one small issue with the food but was taken care of right away. I have only great things to say about Pine Ridge and very thankful for everyone there. read more

I would not recommend this facility to anyone. The level of care is completely unacceptable. The nursing staff neglects residents, leaving them in soiled diapers for hours and failing to respond to call buttons in a timely manner. Communication is poor, and management is dishonest and manipulative. Instead of addressing concerns, they attempt to intimidate families into keeping their loved ones there. Additionally, I noticed serious issues regarding medication handling, particularly with pain medications, which raises major red flags. It's deeply concerning that a facility responsible for vulnerable individuals operates this way. Based on my experience, I have filed a formal complaint with the appropriate authorities. If I could give zero stars, I would. Please do not send your loved ones here--they deserve far better care. read more

This place discharged my husband, who is in a wheelchair, cannot walk, is incontinent, to an independent living facility where his room was on the second floor. They allowed him to take two trash bags of his belongings. The transport service delivered him to the facility and threw his two bags of clothes, etc., onto the sidewalk, breaking them open. He was not even able to get his clothes into the building. He had no way to get to his room on the second floor. After spending all night downstairs, he had urinated all over himself and the wheelchair, having no way to go to the bathroom by himself and no one to help him. He was hungry, having no way to get something to eat. I called the social worker twice. She said she would call him. I don't know what happened, but somehow he ended up with a cardiac episode and found himself in a hospital. This is either elder abuse or abandonment. read more

Hell hole. Waft of poop in the hallways. Unhealthy prison food. World's tiniest television. This facility will dispense medicine, hook your loved one up to oxygen, take blood pressure-- but your loved one will get virtually no service for 13 hours at night and will wind up with a bed sore. Call 911 and get your loved one back to the hospital. read more

I am an RN. I will not go into the details, but my mother in law was horribly managed in this sham of a nursing home. We made a formal complaint to state regulators. They investigated and fired all the administrative staff and fined the facility. This was many years ago. I am sad to see the complaints keep coming in. Looks like not much has changed. The nurse caring for my mother in law on the fateful day of her passing fled to the Philippines to avoid being investigated. Please look anywhere else. read more

I ended up there for three weeks after a UTI and it was horrible. I had only been in a rehab. center once prior to this (Kindred in Greenbrae), which I thought was terrible at the time. In retrospect, it was substantially better than Pine Ridge! They are understaffed, most employees don't care and the food is deplorable. My family tried to get me moved to Kindred on Smith Ranch Road, but they refused to accommodate this request. The PT personnel are good, but understaffed and roughly 1/2 of my sessions were cancelled. I can not imagine worse overall service and feel they should be shut down! They also cater to the homeless and drug addicts. read more

Kaiser erhaben my mother there for 3 weeks. Deplorable. Horrific smell. She was a bit confused so they had a monitor on her wheelchair in case she rolled out the front door. Well, she did; all the way down the hill into an intersection. She was so distressed and in shock. Lucky she didn't get hit by a car. Another time I went to visit, a real snake was crawling around inside the building on the polished floor. Avoid this place at all costs! read more

I reached out to Pine Ridge 6 times over a 12 month period for consideration of my mother at their facility, and submitted her medical records 4 times. Along the way each time I called to follow up I was passed along to someone new who would "have to look into it" or to someones voice mail and although I provided all my mothers info each time including all my contact info, I never received one call back. Yesterday after 12 months of attempts, I reached Mary Fay (whom I'd spoken to a month prior and resubmitted all paperwork to) who said she didn't remember me but that she'd look into it (again), and would call me back the same day, but never did. Today I reached her for the third time and when I told her my name, she plainly replied "we have no beds"..."please try somewhere else". When I asked if we could be placed on a waiting list for an open bed, she absentmindedly said we could call back in the future. When I asked if it would be possible that I could be contacted when there was an availability, she absentmindedly said "sure, thank you"...and was about to hang up when I felt sure I'd never hear from again her so asked if she had my name & number since I hadn't heard from her yesterday...and she absentmindedly said "I guess not"...at this point it was so clear that she had no intention of following up with me now or in the future, so I asked if there was someone else I could speak with and at this point she replied "We cannot meet her psycho social needs". I asked if this was the actual reason for not considering my mother, and she admitted it was. She admitted it had nothing to do with bed availability. I asked her to send this to me in writing, and she promised to, but nothing has arrived so far and I doubt it will. While I'm glad to finally know the reason no progress had ever been made with our application, I'm disappointed that they felt so unaccountable to providing a simple clinical response before now. It has wasted our time and effort, and could have easily been avoided, by the courtesy of a brief reply. I guess it's good to see this type of inconsiderate and unprofessional behavior before you entrust them with the care of your loved one. I'll keep looking. read more

So many staff but no one listens. My Mom was there for a little over a month and she only had one shower. Instead they gave her benedryl because she was itchy. She also has diabetes and they fed her terribly. The whole time her blood sugar was high and they wouldn't listen to her as far as giving her the right amount of insulin. I even looked up the chart on my phone and saw that they were giving her less than what was needed. When she first arrived, they put her in a room with a junkie who screamed at her whenever she was in need of her next fix of methadone. Once when I was there she was given her methadone and nodded out sitting on the edge of her bed. She almost fell over but my friend who was with me caught her. My Mom said she threatened her that she was going to call her drug dealer friends to come and kill my Mom. This is completely unacceptable. That woman belongs somewhere else, not in a care center with the elderly and patients recovering from major surgery that are unable to defend themselves against crazies like her. Also when my Mom checked in, she gave them the meds she had in her purse so they wouldn't get stolen. Her meds were missing when I went to pick her up to take her home. Also, she had surgery on her right leg and wasn't given any physical therapy afterward until I demanded it and that was the last few days she was there. Now her leg muscles have tightened in such a way that her leg bends outward at the knee so badly that it looks like it could snap from underneath her. This facility should be called: Pine Ridge Doesn't Care Facility! I also called an ombudsman and no one returned my call or visited my Mom. I think they are in cahoots. read more

My father has been there for over a year now! I never thought I could be more disgusted and disappointed with a care facility! I could go on and on about everything I HATE! There is not one thing good about them. My biggest problem is they don't care about the patients at all. Every time I go in there, they are ignoring yelling and screaming patients! I have called them and told them I am worried about my dad and that they need to check on him or he needs to go to the hospital. They never did a thing about it, they even told me they would call me back and they never did. Once I called them again to get an update, they always say "he is fine". Twice now, after I have called and they told me he was fine... he was hospitalized for weeks. I have bought my dad hundred of dollars of clothes! He maybe wears them once or twice before they go missing. I even write his name on the tag and somewhere on the outside so it is clearly marked with his name. Sometimes when I come to visit him, he is wearing someone's clothes as well. I have talked to them about it asking to please be sure they double check that he gets all his clothes back and nothing has changed. Clothes aren't the only that have gone missing either, his phone and the charger were stolen from his room and when I had the phone shut off, it magically appeared back in his room. The food they serve their patients is a disgrace! If it isn't the same food everyday, it is horrible left overs. I don't except them to cook a five star dinner for 100 patients but at least make it edible or enjoyable. My dad complains day after day about how horrible the food is there. That when he was in the hospital they had better food. I have found pieces of non-food products mixed in with the food! Most of the time the food looks like someone already picked through it! There is so many other things that are wrong with that place! I could write a novel of all the horror stories my father has told me and what I've seen there. read more

We are responsible for an elderly friend (95) that is in Pine Ridge. We have been happy with the care and service she and we have received. The physical therapy has been excellent for our friend and she is worked hard and likes it. Management has been responsive to our questions and concerns. Only negative is the parking is tight. read more

I think Pineridge nursing care needs to be shut down and should've been a long time ago. They are so unprofessional and unorganized. They placed my grams in a room with another woman and everyday she verbally abuses my grams in Spanish. She stole my grams tv remote cuz she kept telling my grams to turn it down but she is hard of hearing. She slams the big heavy door on her bed to wake her up. They need to be dealt with asap read more

If you care about your loved one - in my opinion avoid this place. Staff is nice enough but management of medications, compliance with California State Law, deep care of patients takes second seat to their desire to cut operating costs and increase profits at the expense of their patients. My 54 year old brother who is deaf can't speak or hear had a stroke back in May. Before being released from the hospital we could not find a nursing facility who would take him in. I felt like he was being discriminated on because he is deaf and they could not accommodate him. Pine Ridge stepped up to the plate and took him in. One of the therapist there knows a little bit of sign language and that made my brother feel more comfortable. It was rough in the beginning for him trying to get used to this whole new environment. He did not like the pureed food in the beginning, but now that he is on solids and he loves it. He is starting to open up mingling with some residents and staff. I visit him three times a week and every time im there staff seem to be doing their job and acknowledging their patients. The only complaint I got so far is the parking is tight and lately they have been doing a lot of construction on the property. read more

Easy to find, I didn't have any problems with my GPS:) Staff seems very nice, clean & very helpful. As soon as you go in you don't smell (pee pee) like other places when you first walk in, this is a BIG PLUS. My grandmother likes it specially there lunch & dinner time, but breakfast Mmmm... ? This coming December 20,2014 will be two years for grandma living here & so far she likes it, this is very important for me & our family that grandma is happy were she is. The care may be ok,but communication is terrible. I tried to fax them on two different numbers. None went through when faxed from fed ex and ups. When I called to follow up, it was have you tried this number? This was over a period of a week. Our attorney tried to do the same with 3 different numbers. The response was always, sorry try again. When I asked to speak to someone in charge. They were gone for the day. I would not want to risk my fate at Pine Ridge Care Center. read more

I'd give this place 1 star just because of the atmosphere (noise, smell, some of the staff) and this nurse who has really bad customer service! I can't believe she is a supervisor too and as a matter of fact RN, I rather die if she was my nurse but 5 stars to this one nurse that checked in my significant other. She works swings, I always forget her name but she's the sweetest and warm person in the whole building, she made sure that meds, food and other requests were given asap. I heard from some of the nurse assistants that she will be resigning soon, if i was management, I'd offer that nurse a much more better position because those are the ones you're suppose to keep. My son was taken to Pine Ridge for further physical therapy after a horrific car accident. He was put in a bed and left there. He had, among other things, a broken pelvic bone and broken femur so couldn't get around without a wheelchair or a walker and was not to get out of bed without help. He could see the call button but it was out of reach. After 45 minutes he finally Googled Pine Ridge Care to get the phone number and called the front desk. He said he was brought there, placed in a bed and had seen no one since he got there. He needed to use the restroom. Finally a nurse with a bad attitude came and gave him the call button. He arrived there around 1:00, was supposed to receive his pain meds at 2:00 but was not given any until 7:00PM. We came to get him the following day. It was a horrible experience for him. read more

My mother was there for years. I would need to write a book for all we went through. It was an excruciatingly horrible experience. They gave her drugs for years against my express orders, then when I found out and told them to stop, they took her off cold turkey w/o telling me this could harm her. It could have killed her and when she acted out in withdrawals, they called the police and had her taken away in handcuffs - at 94 yrs. old. Another time she fell and broke her ankle very badly on a Friday. They told me that if I did not take her to the hospital, she would have to wait until Monday. Since I could not possibly transport her with such an injury, she did not get to the hospital until Monday, where she had to have surgery and a plate put in her ankle. They did not get her the rehab she needed after this and she never was able to regain full mobility. She was put on a diet plan and no one ever checked to notice she had lost too much weight. She had a small stroke walking with me by the front desk and began sliding to the ground. I yelled for help and the nurse told me "she is faking it" and did not help us. Her eyes were bright red and inflamed and I was told "she has always looked like that". They would constantly want to drug her for crankiness when each time I pushed them to check for bladder infection and that was almost always the cause, but they never checked unless I pushed them. They just wanted to drug her. Every gift I ever gave her, was quickly stolen. I could not fix her area up, as they stole everything I brought in. The state decided they were at fault for all my mother's missing things and they refused to compensate us. Most recently they called to tell me she was unresponsive and it took me 4 hours to get them to send her to the ER. They never gave me her personal effects, despite my almost 20 requests. They never accounted for the money they received in her name and much was missing at the end. Feel free to give me the name of a good attorney. They should be shut down. read more
