My father resided at Maple Ridge Memory Care for three months. In that time he lost 12 pounds, his teeth began to show decay and he went a total of three weeks with only the clothing on his back as they lost the rest of his clothing. They routinely forgot to feed him or fed him with food in closed containers without silverware or food cut so he could eat it. He went over one month without an attempt at a shower, teeth brushed, bed changed and his room cleaned. There was regularly feces and vomit on his walls and floor. He was forced to use a towel when toileting as they did not keep his bathroom stocked with toilet paper. Despite significant feedback to every staff that would listen these significant concerns remained. Maple Ridge robbed my father of his dignity. Do not send a loved one to this terrible place. The head nurse, Heather Lambert informed me she "shoves" his medical record into a drawer with everyone else's and never looks at them. Happened upon the negative review from John...2018. My Mom has been at Maple Ridge since shortly after opening as Spring Village in 2016. There were hiccups at first as with any start up. Since then the ownership has changed...therefore the name change. I can only say (as an RN) that I am consistently impressed with the level of care and the genuine CARING of the staff. You would be hard pressed to find a better facility for your loved one. read more

I would like to very much know why Yelp has only chosen to showcase an unfavorable review of Maple Ridge. My 61 year husband was accepted by Maple Ridge. He was young, healthy & very agitated with late stages of some kind of dementia. I was unable to locate a facility that would even consider him, given his age, etc. Maple Ridge allowed him the freedom to walk, walk, walk - the man was able to walk 5 miles 10 days before his death. Although my husband was only there for 2.5 months it was at a point where it was unsafe to have him home & my family & I could no longer give him the 24/7 care he needed to keep us all out of harm's way. They worked very hard with him to make him feel comfortable - no easy task. He was challenging to the staff & other residents - but this was the place for him. A nurse (Peggy) made a special outfit for him so he would keep his clothes on & maintain dignity. Another resident's wife also made him several. He formed a bond with Angela the housekeeping supervisor & he allowed her (& she did) help in his care - amazing. The entire staff could not have been more compassionate & caring. Towards the end when Hospice was called in - they were extremely accommodating. I was allowed to stay with my husband during his final days - we were able to enjoy each other's company without the stress of constant caregiving. I was with him when he passed - stroking his head & holding his hand - it was an experience that could have been terrible, but was very peaceful & loving. Maple Ridge Memory Care, Formerly Known as Spring Village: Negligent, incompetent, untrained, understaffed. And those are the only positive points I could come up with to describe management at this facility Before leaving your loved one off at this facility, please do your homework. Read all Spring Village's state investigations that are too numerous to list in this review: http://dlp.vermont.gov/search/node/Spring%20Village. Their current Executive Director has tremendous difficulty telling the truth when speaking to families. Spring Village lied to the families with promises made in writing (contracts) They promised to care for residents, no matter what level care needs they require. As of June 2018, this facility has evicted some of the most vulnerable residents, and, are now trying to evict others In a period between 2017 and 2018, several residents died as a result of falls. This facility's management would not say a word about these deaths, other than to post a memorial picture of the deceased in the lobby. As family members, we understand medical privacy laws prohibit the facility from discussing why a number of residents have died, but, I was hopeful they would add enough staff to care for the residents they do have, or, change how they care for residents. Lots of promises since opening, but no corrective action. Not enough staff to monitor residents in confusing hallways. No fall prevention plan in place. Chronic, criminal, low levels of staffing. No wonder many residents have been injured from falls. Our father had unexplained bruising when he first arrived. We could not tell if it was from a violent resident, or one angry nurse that used to work there. Every time I visit my father, there are new staff faces. As soon as you learn staff names, they leave. This facility has gone through a shocking number of Directors; executive, activities, nursing, care providers, maintenance etc. Another tragedy is the number of wonderful, hardworking, care providers that have gone through the facility doors in the short time they have been open. Management treats support staff like disposable briefs. I once witnessed a temporary medtech ask a SV cleaning person to help identify a resident so she could make sure she was administering meds to the correct resident. On more than one occasion, they "forgot" to feed my father his meals. Family members are forced to get facility news and up-dates by way of mouth from other families. Management would rarely up-date us with any useful information. Lack of personal care for residents (bathing, nail and hair care) Lost and misplaced mediations Running out of medications No towels in resident rooms No soap Feces stains on resident's floor; Director of cleaning staff, claimed she scrubbed them many times, but "could not get them cleaned". I have a suggestion for this Director, how about less smoke breaks and google, or youtube how to clean. How about less time cleaning the facilities Executive Director's private bedroom on a daily basis and spend more time caring for the resident's rooms? Feces on resident chairs for weeks while the Maintenance Director ignored requests from family members and care providers to please clean. Feces on resident walls and bathroom for several months. (we took photos if you would like to see any conditions inside) No fire alarm plans to evacuate residents from a locked facility The promise of baths when we first moved in 2015. He has had ONE bath since being there. He now may get one shower per week if he is lucky. At times, one staff member for 12-15 residents in the dining room; if a resident needs to use the restroom, they are made to wait. They are on to their third Executive Director in the short time they have been open. Each one, including the current, lack experience to manage a facility such as this one. Maple Ridge took good care of my father who suffered from early onset alzheimer's. They made sure that he was safe and well cared for. Everyone from the housekeeper to nurses to med tech supported him in his time there. When he was passing they made sure to keep him comfortable. They also took great of my family as we stayed with my father in his end days. I am very grateful for all of their care and support. read more

It is important to know that this facility is now under brand new ownership and directorship. This inside view gives above average ratings for the following reasons. Training of new aides, ongoing meetings and education, kind, considerate and caring staff who treat your family like their own family. Communication is huge here and the director listens to and gives her time to each staff, no matter what level from the ground up. There are meetings for aides and nurses to communicate together and inform each other of each and every change in order to provide the resident with the very best care. A superior activities department that is the envy of every other facility. The Chef is superior, food excellent, RNs are staffed to supervise each shift around the clock and care is given to the minutest details or differences in behavior, skin changes, physical needs, etc. to protect your loved one. Every new facility has "growing pains" and this one is flourishing now, give it a try! read more
