When i was there the staff was helpful and caring. Sometimes they were understaffed and couldn't be in the rooms right away it wasn't their fault but they did the best that they could. I remember one time my pain was so bad I pushed my button for help they couldn't come right away so I was waiting my roommate climbed into her chair and raced out and said hey she's in serious pain i think she needs to go to the hospital they raced in examined me and immediately called 911. When I came back they apologized for not responding sooner. I said you guys did the best you could and I appreciate that. checked out AMA because the care was terrible. long waits when turning on call lights. most of staff was not qualified to be working there. floors were sticky and even had blood on floor next to roommate bed which was close to bathroom. rude and ignorant workers. lacking in training and compassion. meds. not given in timely manner. even woke one morning and had pain med. sitting on bedside table. never offered a wash cloth for cleaning face and hands. so much wrong there I think State needs to check someone in undercover an experience first hand. I saw a lot of violations that would be a health concern. would not recommend for anyone. read more

Horrible place! Staff is mean, unwilling to help patients or family. They wouldn't answer their phones for hours at a time. They lied many times to me about health issues. The nurses wrote events that happen so sloppy that no one could read them. They undermined the insurance company, lying saving they were not getting paid for pt services when they were and not giving patient pt. My husband had a stroke, and they assured me they would be able to rehad with him. Instead, after 6 weeks, he almost died there from their neglect. The care he received was totally unprofessional, and their was never a dr available to discuss his case. read more

Hearthstone staff has the biggest and most compassionate hearts ! Their expert team of nurses, CNA's, therapists, housekeeping, transportation, dietetics, hospitality, activities and admin work cohesively to bring excellence to patients. Each one is a gift ! My loved one has been there for a few months and is THRIVING! He is more independent and confident than we could have imagined after suffering a severe stroke over a year ago. ‍ His speech and sounds are improving with lots of giggles and positive communication strategies, even when frustrated. Additionally, he is motivated to get out of his wheelchair and attempt walking more frequently. His confidence is soaring with this wonderful team of professionals that surround him 24/7. If I could give less than one star I would. There are a handful of competent people there but the terrible ones far out weigh the good ones. mom was rehabing there for 21 days, she lost 23 lbs. I waked in to my mom's room all of her clothes she had were dirty and mom was unkept. All of her toiletries had been stolen, no brush, toothbrush etc. I complain to the nurse she became angry and said we don't do laundry here. they had been doing the laundry for 2 weeks and had us put names in all her clothes so when THEY do the laundry it would not get lost. mom needed a series of tests prior to moving to her new home. They said they gave them then they couldn't locate them in the computer so it was going to hold up her transfer for 17 days. Mom had a maintenance drug that is crucial that she gets, it was not given to her for 21 days and an anxiety pill to be given nightly. she only received it for 3 nights.This med is not supposed to be stopped abruptly. My brother went to ask someone to speak to her but she was too busy dealing with a murder quote unquote. mom developed severe Alzheimer's symptoms in the 3 weeks there which is common for the elderly. In all fairness she is 88 years old and had memory issues but they went from a 3 to a 9 in severity. This part is trivial but the residents were sitting around in an exercise group each resident in the group were given a bean bag and the leader would bend over with her rear end in their face and asked then to throw the beanbag at her butt. Not appropriate. I was directed to be there at 10 to pick her up. I asked several people what I needed to do or whom to go to for discharge no one seemed to know. Her nurse said I don't have discharge orders for her. While we are waiting someone comes in to tell me she has bad news they have lost some of the proof mom needs to move in to her new home. the nurse hands me mom's meds but there are only 3 she takes 5 everyday meds. the two omitted are crucial for mom's mental health and kidneys. Mom had been in her new home for an hour and a half when the new place received a call stating that mom will be discharged later that afternoon. this is a locked facility that had no idea she had been gone for an hour and a half. I would not put my worst enemy in Hearthstone. read more

My loved one has been in this facility for over a month and has not been given one shower. He/she are getting skin issues due to the fact they do take care of them hygienically. Do not put your loved one in this facility. It is so very dirty and some of the CNA's appear to be a bit rough and rude with the patients I have seen them interact with. Except for the friendliness of most of the staff, this place is one step above Auschwitz . The food is horrible they give you small portions and it gave me heartburn for days which I never had in my entire life. One morning breakfast consisted of one donut and two sausage links . And I'm diabetic and they still give me a donut? Absolutely horrendous place I would never stay there again if I have the choice. Then when I left they gave me a set of crutches which I needed and had requested a week before and when I got home to use them I realized that they were maxed out and about a foot and a half too short. A loved one had the unfortunate luck to spend an extended time in this facility and she is now paying the price. I'll never be able to say for sure what happened to her, but her current medical team agrees that neglect played a direct role in her health decline. Make of that what you will. Beyond that, the staff of Hearthstone was extremely unhelpful in basically all regards. I understand COVID guidelines and restrictions affected our access to the facility, but the staff did nothing to help problem solve with us to get our loved one what she needed, including access to an attorney. We were unable to directly communicate with anyone except the billing department (eager for their payments). When the main phone line was picked up (rare), we could expect a long wait time only to end up in the wrong department. If you or a loved one is looking at Hearthstone, DON'T. read more

My elderly, handicapped sister was transferred from Renown late yesterday afternoon to Hearthstone to start physical rehabilitation. As her guardian, I called and asked what they needed me to do. The receptionist advised me that the social worker would call me. My sister spent the night and when I arrived in the morning to check that she was on her way to fully recovering from hip surgery I learned that they had already transported her back to Renown. She now sits in the ER at Renown. I was not called to be advised of what they intended on doing or why. They decided that they could not help her AFTER the transfer. Hearthstone should have been aware of her needs and condition from Renown. They should have advised me that they were rejecting her by a simple phone call. read more

My mom ended up in the hospital 10 days ago. A week ago, she was accepted into Hearthstone of Northern Nevada Rehabilitation Center. Coming from California, I didn't know anything about the Rehab Center. Well, I'm glad my mom is there. It's a really nice place and everyone there is not only professional but nice. The therapists are awesome! My mom is 92 and getting great care. I thank the whole staff for the care they're providing. read more

Almost killed my father in law went to go see him he was so doped upset he couldn't talk was in bed only 1 young girl was in charge she didn't know anything . Had to call an ambulance to transfer him to hospital er the er Dr said he had a liter of fluid in his bladder had a Utamaro track infection also had nemonia and Covid er said he would of been dead by Monday if we left him at hearthstone instead of rehabilitating him the drugged him and left him unattended with no help . They weren't aware of all the things that were wrong with him that he contracted while he was at there rehab place he went down hill from the day he got there . Tried to talk to man incharge a week pried to the time we took him out. The person didn't want to talk to us on his condition he said he didn't have time went to his office he was doing nothing but sitting at his desk still wouldn't talk to us said he would call us tomorrow. Never got that call . I wouldn't recommend that place to anyone stay away. read more

My review is PRE-COVID, I was just waiting for my family member to be out of there while working more official avenues. You know those movie scenes of elderly hospitals that have catatonic folks staring at whatever is on tv, some drooling on themselves for hours? That's this place. This is where you dump a family member that you just don't want to deal with. My family member's roommate feebly called for water several times in my brief visit to the point that I went and found someone because I wasn't qualified to administer. The "care" provider was completely lacking in bedside manner with this person. My family member sort of shrugged off the situation as he was a hospice level resident. There were stains of some sort all over the walls in the room, the whole place stinks of impending death and the cafeteria area was piled with dirty dishes hours after lunch ended. Most of the "care" staff seem to be detached, though there are some standouts that do their best within the limits the administration of this wastehole has given them to operate under. Washoe county nor the state seemed terribly concerned, but if you love your family member in need of care-please keep looking. This is not somewhere you go to "rehabilitate", this is where you go to die to allow everyone but the dying to go in peace. read more

it's a horrible place, they give them expired donated food, my grandpa has fallen over 2 times already and no one assists him, the workers only talk and when we say he needs to be cleaned or help him onto the bed and so he ended up falling onto the floor, and the physical therapy said he was okay yet they didn't help him because he was already off work, the workers name was alex, my grandpa has been there for over 3 weeks and they haven't cleaned him, showered him, they don't care for them whatsoever, the whole place smells like pee and ive already talked to the employees and they say is that they're in a critical situation because they're in need of employees because of how they act all the nurses are rude with the older people they don't believe them, if the employees changed the way they acted, i think it would go smoother and nicer with treating my grandpa and everyone else. read more

This place is a nightmare! From the top down. My husband was 54 yrs old and a retired prison employee who had a major stroke. We wanted their care for his rehab and they almost killed him and thank God for physical therapy staff telling me to get him out of there or he may have died because of the care he was getting there. My son has MS, bipolar and is wheelchair bound. He needed to have rehabilitation because a recent illness left him extremely weak. His first night, Monday, at Hearthstone they served him food he could not eat without assistance. He did not receive assistance for 30 min resulting in eating his dinner cold. On Thursday that same week I visited him and he was gaining his strength back. On Friday afternoon I received a phone call the he was insisting on going to the emergency room because of weakness on one side and drolling. I ask the nurse isn't that the signs of a stroke? When I arrived at the ER the doctor said my son had a UTI and sepsis. My son obtained the UTI because he needed to go pee and no one came to help him and he ended up peeing on himself then had to sit in it for a while before a nurse came in to help him. I would not wish the behavior and lack of concern the nurses at Hearthstone have on my worst enemy. read more

I would say if you have loved ones do not take them to this place as their understaffed, the bathing room floor isn't disinfected after each patient is bathed along with storing trash cans full of dirty diapers, meaning full of feces which can be smelled while bathing. Then your food that doesn't meet the health department regulation by using hot plates to keep the food hot or they serve food ice cold, burnt, undercooked, or serve food in styrofoam containers where the food is plain cold. Or the food is rock hard due to it being over microwaved to reheat the cold food. I've talked to the kitchen manager and upper management and problems are still persistent here. It's just not the food which is responding to call light hasn't improved as well or sleeping on the over 2 and a half weeks. It seems CNAS meaning 1 to cover more than one can handle. The big picture is one cannot locate their corporate office to file a complaint so, now I'm left with filing with the state board along with contacting the local news station. read more

I'm really not impressed by what I saw. I went to visit my cousin who is recovering from surgery. When I reached the secure access and pushed the button for access, no one replied, I tried again and still nothing. Finally some EMT's were exiting and let me go in. Once inside, I wandered til I found an empty nursing station. I text my cousin for her room number!! She mentioned that the food is terrible, no fresh fruits or vegetables. Everything is canned, and she's been blessed to have friends bring her a few bananas or grapes. What about those who don't? She waited on pain medication for over an hour and was very uncomfortable and anxious due to her post op condition. When I left a few minutes later a bunch of the nurses and cna's we're watching tv together in a group area just outside my cousin's room. Is that why no one could dispense the meds? It's a sad place to be. I pray I never have to stay there. I'm pretty disgusted. read more

This is by far the worse place you can take fir loved one. If you have the option really do research and select a different rehab/resting home. This place gave my grandma minimum care. Let me elaborate it first started when my grandma was sick with pneumonia when she was released from the hospital she needed therapy and rehabilitation. The hospital automatically selected this one. We didn't know better this was the first time experiancing this process. I would pop in all the time which was nice my grandma was my everything. There were multiple times my grandma had an unbearable stentch which was from not being bathed/showered. Which according to the "log" she was consistently showered as per the norm schedule every other day. So i would shower her with my fiance because there was no way she was being hygenically maintained. I would also have to personally request to have bedding changed and her gown because it would get very obviously dirty. But the straw that broke the camels back was one night i couldn't sleep. So i decided to pop in and bring my grandma snacks. It was probably 10-11pm at night and i as i walk in my grandmas room she's sitting up wide awake i notice her gown its pretty soaked. I ask her what happened she said the guy that was checking on her " accidently "spilt" water on her by knocking off her table on to her... hmm not sure how she was so drenched from that but ok she said he was getting a change of clothes and bedding, so i started to chat with her and show her what i got her before i know it 20 minutes passed. As i checked my clock when i walked into her room. So now I'm very upset! I go out of the room and i see a lady on her cell phone just chatting it up mind you my grandma had off and on pneumonia. So the sitting cold drentched clothes and bedding probably wasn't good! I go to her i say well wheres the guy!? She saud what guy? I said the guy that's taking care of my grandma in room xxx? I said my grandma has been waiting for a change of clothes and bedding for 20 min that's I'm aware of but who knows when this really happend. Because my grandma never wanted to get anyone in trouble despite the situation. She then looks at me panicked the desk lady because she could tell i was beyond angry and said the guy went home for the night. I couldn't believe that this kid just decided to leave an elderly in that situation. She immediately attended to my grandma but what would of happend if i never popped in? I requested an immediate meeting with whoever was in charge and two ladies met with me and i was immediately able to get my grandma transferred to life care in damonte which was such an amazing place. Yes a longer drive but no comparison. Also morning star is a great place. They moved my grandma with no hassel which is usually hard due to insurance etc.. But they knew this was really bad with everything i had documented. So if you love your family member keep them far away from this place. Not only that they literally serve slop for food i couldn't believe how gross it looks. And its very run down and un kept. The nick name for this place in the hospitals is tombstone for a good reason. Also they hire with no experiences necessary for 10 bucks or less what kind of quality care do you think your loved one will get? read more

If this place could be given less than one star...I would do it! From the moment you speak to the surly receptionist "Dana" to the moment you realize your loved one is in hell... Anthony Alexander is supposedly in charge and always missing in action...never returns a call. They have a few good nurses in short term care (like Wendy) Long term care is a nightmare!! They dont give a damn!!! I have reported them today due to isolating my exhusband as in: NoPhone in room and over 45 minutes wait, calling again and again and again to get thru!! They promised transportation to an SSI Dr appt made month in advance...day of appt they said van was busy. They have withheld bathing him for over 7 days. He lost so much weight due to not being fed...They have withheld (or stolen???) pain medications. They refuse to give him physical therapy. I have heard them yell at a nice little old lady while waiting on phone transfer. This place is rotten. I hope the investigation will rid the place of the rotten folks that are there now caring for helpless victims. I will let you know if anything changes.. (But 1st time I heard of Hearthstone was when they treated my mom with alzheimers horribly in 2001. Then another gal I homecared for called it Headstone in 2005. Funny, but I thought I had already written this review, but found Hearthstone to not have any reviews all of a sudden. Yelp? My mother-in-law got placed here for "rehab" after an ER visit. She was living on her own successfully for many years before entering the hospital because of some health concerns. Turns out she had a UTI, which can cause symptoms that mimic dementia if left untreated long enough. She did not have dementia - she was perfectly fine mentally after the UTI was treated. The hospital was concerned with her light eating and wanted her to be stronger physically in order to go home. The times I visited Hearthstone, not one single staff person greeted us, acknowledged our presence or checked on her - never. We encouraged her to walk around the facility because she had to gain strength to return home, but she told us they would not let her get up by herself, and no one came to get her out of bed. The room was dark and not very welcoming at all. I personally would not have wanted to be there. She was transferred back to the hospital because "she wasn't progressing." Well, I wonder why. Hearthstone was the second stop on the way to her death - she ended up in her hometown nursing home, fell, developed a blood clot, and passed away on their watch. Something is seriously wrong with the system we have to care for our elders. I believe the other reviewer was correct when she said they are too understaffed to properly care for their residents. read more

I would not recommend this facility to anyone. I had problems when trying to provide music to their residents, especially to a man who had no arms or legs, and I was completely let down by their staff. In my opinion, a good administrative staff includes people who don't lie and who don't put up with liars. If you listen to their promotional verbiage while on hold I will tell you what they're saying is not true. They do not care for their residents as much as I think they should. Edna, the activities director didn't even know the name of the limbless man. She told me there were too many. Her boss, Susan, confirmed he was the only one so Edna lied. She asked Edna why I had not been allowed to come back and play after the time I was there and learned that the armless, legless man had a favorite song - as I was going to learn it - and it was relegated to budget issues. That is a lie. I would go to places like Brookdale on Plumas in Reno, 5-star Premium on Plumas as well, and even Rosewood. They have an excellent staff that cares. read more

According to the social worker that I spoke to, Hearthstone has a 5-star rating from Medicare. I can tell you that this facility is in a nice area of Sparks. Hearthstone is an older building and the staff seems to be less informed than the staff at Life Care Centers of Reno. I had a quasi-good experience during my tour but I do think that Hearthstone is the #2 facility in the area (when you consider the facility condition, location and staff.) read more

This is the worse place ever for taking a loved one to. My mother had dementia, suffered a broken ankle, was in a cast and was placed in this facility for physical therapy. Unfortunately, due to her dementia, this was the only place that would take her. I read reviews on all the rehab places in the Reno/Carson City area and only 2 stood out as excellent. The other reviews, including Hearthstone, were terrible but I had no choice. My mom was placed in Hearthstone's memory care unit on a Friday night. I live about an hour away and could not get up there until Sunday to visit her. I arrived just before lunch time and stayed with her for a few hours. I told a staff person that I had some questions to go over with her and she told me it was her lunch time and basically left. Lunch was served to my mom and another gentleman. Not one staff person could be found for over an hour. My mom needed help cutting her meat and seeing her food (because they did not remind her to put her glasses on, not did they have her put her hearing aids in). Since she was in the memory care unit of this facility you'd think they might assist these people better than they do. I had brought some clothes up and put then away in her room. The bed was just a few inches off the ground, there were no bars on the bed nor was there a "motion pad" to detect if she tried to get out of bed. My mom could not remember that she had a cast on and tried to get up and help me several times while I was there. I told my mom I would be back the next day. When I left I searched for the physical therapist but no one seemed to be there on Sunday. I got a call at home around midnight on Sunday that my mom had tried getting out of bed to go to the bathroom, fell and ended up dying. She wasn't even in there a full 3 days. As one review stated "if you care about your loved one, don't put them in here". His loved one died while in their care as well. I hired a law firm to investigate this place. They said they "could not find ENOUGH evidence to show that they were not following the law but that another firm might be interested in pursuing this." I did not pursue. The State of Nevada also did a surprise visit, after my formal complaint with them, and they came to the same conclusion. They went during the week, not the weekend where staff seems to be at a minimum. I would NEVER recommend this place to anyone. They don't give a damn about your loved one! read more

"If you don't care about your loved one this is the place for you" to start off the medical aids are not qualified to babysit your family pet. They possess no medical degree of any kind. They lack any compassion for their patients. Had I known this I would have pulled my wife out much sooner. After 2 days of no food or liquids I demanded she be transferred to a proper facility. It took them 2 more days to get her transferred.4 days with no food or water. She passed away 4 days later. This place should be renamed "Deathstone" read more

My father required 5 weeks of rehabilitation therapies and skilled nursing care at Hearthstone of Northern Nevada during May-June 2018. I visited him every day and got to know many of the staff and the entire facility very well. There are not words to express the superb quality of care and therapy, professionalism across the board, thoughtfulness, ease of communication, and many examples of individual initiative. So many kind people that exceeded expectations and earned the highest marks: nurses (Tatiana, Kathy, Amy, Starlight and Joyce!! outstanding), therapists (Kyle, Liz, Jennifer and Dan!! best in the business), CNAs (Gabriela, Lawrence, and so many others!! there is nothing they cannot or will not do), housekeeping staff (a great big shout out for the prompt and excellent responses to emergency messes - everything was maintained so clean), nutritional staff (always ready if a patient/resident needs a cup of coffee), activities staff (such fun - they even got my father to dance on his last day!), social workers (especially Jackie, OMG!), case worker (Judy - way to go!), transport (David), administration (Susan - thanks for the help with the peripheral family soap opera! and thanks to many others), and last but certainly not least the excellent physicians (Dr Phillips and Dr Maul - of course they are busy, but they will find the time for your questions). Hearthstone is a nursing and therapy center that takes care of the patient and the family. If you or a loved one are in need, I cannot believe there is a better facility in all of northern Nevada. Watched my grandma deteriorate and finally pass in this awful place. They stole her jewelry, and that was AFTER she fell in the bathroom because the staff was always "under-staffed" for the night shift so no one would come to help her when she pushed her button. She had to wait an hour for an ambulance after she fell. Broken wrist, bruised ribs, bruised elbow. She fell 3 more times before she could no longer get out of bed due to pain. Her ring is still missing and the three people working the night shift "have no clue" what happened to it even though the watch she was wearing ON THE SAME HAND ended up in her drawer and EMT's reported no jewelry on her. Grandma is gone. This place sucks. Dirty, improperly staffed, under paid staff, crappy parking lot, and apparently careless owners because nothing ever gets fixed. But there are tv's everywhere and the front room looks to impress if that matters to any one. read more
