This facility is the worst! Do not allow your loved one to go here. If your loved on is here, please have them transferred. My mother was transferred here and after 12 days and us noticing her heath declining and staff not checking vitals, running labs or the physician ever seeing her, we called 911 and had her taken to an ER. My mom was admitted to the ICU for 2 weeks before being downgraded to a medical floor. The ER told us she was sick and kept saying they couldn't believe she came to them like this from a facility. We told them she was at Forest Ridge and they said "that place is awful" This place was negligent! My mother passed away a few weeks later. I reported this place to the state and they were very familiar with it. This place has had many investigations and fines and is still allowed to stay open. An investigation was opened form my complaint and many issues were noted, fines given and that's all that happens. They found that my mom was not the only one the physician did not and was behind in seeing many other patients as well. They filed bankruptcy, were sold and are now filing bankruptcy again. Rehab/SNF are not held to a standard and it's the patients who pay for it. This needs to get attention and be changed. The elderly can't and don't always have a voice or family to advocate for them. This facility was a nightmare. Had a loved one go here after a major procedure for rehabilitation. So many things happened in the 20 day stay that this is not enough room to type. Staff was about 50/50. Food was awful. Loved one begged to get him out of this place ASAP. If your loved one must go here, make sure you check on things daily. Not recommended. read more

This place is a nightmare and is run by a bunch of incompetent imbeciles. Before you check this place out, know they were fined in 2017 for the death of a patient. The pictures on the website are a false advertisement and a tour will reveal that. Unfortunately, being on state insurance leaves you with little options. I feel so sorry for the patients that do not have a voice and those that rely on the staff because they are neglected. My mother checked in about 3 weeks ago, after being discharged from the hospital and nothing has gone right. The following happened and I'll start with what pushed it over the top and then add other details. 1. About 1 week ago, my mother's insurance denied their claim for physical therapy and other services at the facility. All services stopped all of a sudden. She was not told what was going on. She even asked after a couple of days and because it was the weekend, no one was there to help. She should have been discharged right after that. (I think they are committing fraud and trying to rack up the money). It wasn't until I showed up today asking questions, threatening to file a formal complaint, which I will be filing with the WA state ombudsman and the board of health. No one knew why she was still in the facility or what was going on. I went through 4 or 5 people before Chris looked into stuff. The social worker avoided my mother and never spoke with my mother when she requested information. Today, when I showed up, the social worker stared at us and refused to speak with us. She did not look pleased. I plan to get her name and badge number and file a complaint against her. Finally, they gave her a discharge date in 2 days because she had to get training on her medication. She managed her medication better than them. They need training. 2. She was not given a tour until 3 days into her stay and was not given information about the services she was supposed to receive. No one knew about the physical therapy and that did not get set up until I told her to go to them. 3. They never had the right medication for her. She knows what she needs and when she asked, they would always say the pharmacy did not bring the medication. She missed several days/does. She had to go without her shots (to reduce the clots) and her inhalers. When I picked her up, she eventually got the medication from home. Anyone knows you need to be consistent with medication in order for it to be effective. This could be a liability issue. 4. The food was slop and always looked as though it was slapped on the plate or someone had been playing with it. The food was always cold. She ate breakfast and nothing for the rest of the day. I would go and pick her up to get food or let her come home. 5. The nursing staff was lazy. She would request items, but the nurses never returned back. The night shift was the worse. She enjoys a few young ladies on the day shift that took care of her. Her roommate was not bathed frequently and hair was all matted and smelled. One nurse would sponge the roommate off and wash her hair the best she could and put a braid in it. The roommate is bedridden, but somehow she ended up with a fracture. Every time I go in there, her colostomy bag always looked like it needed to be changed. It looked like she was never changed positions. When you approach the front desk to request information, the staff does not acknowledge you. We waited for about 5 minutes while 2 staff chatted and joked in the corner. 6. Her toilet did not flush and it was a day or 2 before they fixed it. With her roommate bedridden, it must have not been a priority. My mom held it from the time she left my place until I picked her up. Eventually, they fixed it, but it was still slow to drain. So, do your research. Avoid this place if at all possible. read more

My Son was in this facility with Kidney disease and Diabetes. The meals he received were not at all good and the kitchen NEARLY ALWAYS put POTATOES and TOMATOES on the food! This is a NO NO to renal patients. The chief dietician was quoted as saying, "Potatoes are allowed because they are instant mashed potatoes." SERIOUSLY! She has NO clue. Instant mashed potatoes are to be avoided at all costs because besides the potassium they contain TOO much salt. This morning (June 16, 2021) my Son was taken ill and he had to DEMAN they call an ambulance. He is now in the hospital with a serious lung infection. Thanks Forest Ridge you sure know how to treat your patients.......NOT! My advice to anyone considering this facility is to avoid it at all costs unless you want your loved one to have negligent care. read more

This place is awful! Don't come here. Don't let your loved one come here. They are severely under-staffed it takes an average of 20-30 minutes to get a response once you've pressed your call button. The website boasted gourmet food. Pork roast, canned spinach, canned wax beans and canned pineapple all prepared without seasoning is not gourmet. I'm here to rehab from a broken back and server sciatica. Also they have no services like physical therapy on the weekends. read more

If you can help it do not send your love ones here. They are under staffed. They let their patients run around at night when the other patients are sleeping and stand over them a foot from their bed. Very creepy. The social worker never returns your calls. read more

I was there for 2 days and nights. I wasn't expecting the Ritz, but this place was bad. I had to be in isolation due to covid, but the isolation area was run down. Wires and electric cords on the wall with the tv's. My father passed away in a place like this and I couldn't wait to leave. read more

In the last four months this place has gone to complete garbage. The new management who took over has been nothing but issues. They DO NOT have the best interest of the residents or the workers for that matter. They are understaffed because they treat the employees like trash and they find work elsewhere. Those employees have been forced to leave the residents who they have care for 6+ years and management DOES NOT CARE. They are neglecting the residents by not following doctors orders, VERY unhygienic, and do not order medications when they are due to being ordered. I am completely disgusted I wouldn't even give this place the one star but I had to make a selection to be able to even share. DO NOT WORK FOR THIS PLACE AND DO NOT SEND YOUR LOVED ONES HERE UNTIL A BETTER MANAGEMENT COMPANY TAKES IT OVER!!! read more

This is not the prettiest place, or the quietest, but the people who work here really care about the patient and the family. My father was a resident for three months or so, until his passing this evening. The care professionals kept him comfortable, and treated my mother and our entire family with respect and kindness. read more
