I also double-checked the information I'd left them with, as it was copied in the confirmation email they sent me; the address and my phone number were all correct. By 5:30, after no taxi had arrived and nobody had even given me a call, I couldn't wait any longer -- my flight left at 7:00 am. I called and left another voicemail to cancel my cab and drove myself to the airport. I still haven't received a phone call or an email from Yellow Cab. I'm glad most other people had great experiences, and perhaps mine was a fluke, but it's a fluke which is going to cost me roughly $165 ($195 in parking fees versus ~$30 for a taxi ride and tip). Next time I'm going back to the Super Shuttle, which I don't really love -- but they've always gotten me to the airport.
