This is where I filled out my paperwork. There also were a few snacks offered! Michelle was amazing from start to finish. I appreciate they ask before if you want quiet or conversation, so it's not awkward if you just want silence! She was so incredible - I left being able to move my neck and a month later have no pain still! Worth every penny.
