To my surprise, I was told my copay is $45 per tooth. I was left with no other choice but to file a grievance because the dentist office was no longer available to provide an explanation. I waited 30 days & the dentist office responded to the grievance and said they use code D9611 for the Arestin shot. I looked up code D9611 & it does NOT relate to any form of an anti microbial shot.
