It is my hope that this message is received my anyone that's considering placing their love ones into this facility. There's many good and outstanding employees there, but they're being silence by silence by the nursing supervisors that are lazy and incompetence. This facility is being operated under the guideline of race and discrimination, or one can look at as reverse discrimination. There's a heavy occupation of African employee supervisors in the management staff. So all the bad habits, incompetence, negligence performance commit by African staff is covered up by the African administrator, the African director of nursing. While the piss poor behavior of the African staff is condone and covered up by the African management staff, this same management and supervisor treat the African American staff like dirt as if they're beneath the African employees. it's the typical operation that's ran by nepotism, favoritism. Please, seek out other facilities before chosen this poorly ran facility, they're collecting checks from Medicare, social security, and Medicaid and giving nothing in return other than the lowest quality care one can imagine. read more

Be warned this facility is infested with rodents and roaches. My mother is currently in the care of this facility living in these conditions. I saw a roach on my mother's bed, dresser and one coming out of a hole behind her bed. I first pointed this out to a nurse who then told me maintenance was in a meeting. I then demanded someone come address the roach issue immediately. There was no sense of urgency until I walked around telling other nurses what I saw asking for help. Eventually Fred the administrator and the housekeepers and maintenance showed up. I showed Fred the picture of the roach on the dresser knob and his response was," That's not a roach" !. I disagreed and he then instructed the maintenance and housekeeper to address the multiple issues ONLY IN MY MOTHERS ROOM. After they removed the dresser and chest I took pictures in front of the faculty of the mice droppings, mice traps with roaches in it, a pair of someone's old dentures and other trash that was there before my mother was admitted. Several holes in the walls, black mold around the sink and etc. *See pics*The social workers office was not interested in my concerns and refused to give me any contact information for the owner Yitzy. The uninterested rude social workers two women in the office today showed no concern over my mothers living condition. The polite social worker Sabrina was not in the office. Do not send your sickly family members to this place if you want them to be in a sanitary facility to fit there medical needs. The entire facility needs remodeling and intensive cleaning from wall to wall and visited by a large crew of exterminators. Health hazard! Please thoroughly check your loved ones living conditions in this facility. I saw a roach run out of my mothers room towards another resident door but a nurse stepped on it. This place needs to be visited by the health department and OSHA immediately. I feel horrible for the residents living in these conditions and the understaffed nurses working in this filth. This is extremely unhealthy, unsanitary and insane. Take pictures and report Autumn Lake at Bradford Oaks if you have witnessed anything similar. If I could put zero stars I would. This facility has brought great grief to my family with the sudden passing of my grandmother. Their facility is not equipped to care for ANY elderly person. If your loved one needs physical therapy, don't send them here. The therapist told my grandmother she wasn't able to lift her up to get her legs moving so she moved her legs in the bed causing bed soars. If your loved one needs dialysis treatments, this isn't the place for them. My grandmother complained every treatment regarding how rough the nurse was inserting the needle. Another patient complained that the needle slipped out of her arm and the nurse was careless inserting it back in. The negligence of these nurses got so bad that unfortunately my grandmother bled out of her tube causing her to have a stroke and ultimately pass away. Then to add insult to injury, someone stole her Diamond bracelet that my grandfather gifted her for their 57th anniversary. This facility FAILED my grandmother, along with other patients. DO NOT ALLOW YOUR LOVED ONE TO BE NEXT. read more

My mother-in-law has been in the hospital 4 times since being admitted to Bradford Oaks in November. Staff tells you what they are doing for your family member however if they were doing their job my mother -in- law would not be going back and forth to the hospital. read more

I don't have enough time to express what my family just experienced with the Director Samaria Washington. However, I will share this much. Samaria Washington and I did not have a pleasant conversation this past weekend or on Friday because I questioned the integrity of an email she sent out that to myself and other family members. Please note, my mother is a long term resident at this facility. The first issue was the email was not blind copied, just one of the many issues I had with this email. As a result of my concerns and our very unpleasant conversation she decided to have her activity director to contact my husband who was a volunteer chaplain and tell him they no longer needed his services. So now the facility no longer have a chaplain presently in the middle of a pandemic because she didn't like my concerns which has noting to do with him. Her actions raises red flags!! read more

Please DO NOT send your loved one to this place! They treated my mother with neglect for the last six months of her life! They started off professional but turned as time went on! My mom fell numerous times, had black eyes, was IGNORED by the nurses every day they left her with FECES AND URING in her diaper for hours even she told them she had to use the bathroom (I know because I use to stay on the phone with my mother). I always had to call their phones back to back to get them to answer and go and check on her. They barely changed her diapers as they should have. Each time she went to the hospital her diaper was dirty or she had NEW BRUISES!! The nurses take up for each other and constantly lie to families when in fact these elderly sick people are being ignored, abused, or neglected. I went to pick up my mother's items and everything I took up there for my mother WAS NOT RETURNED!! Her items were STOLEN or THROWN AWAY. This is very heartbreaking for our family because from June to September my mother begged to leave she kept saying they were mean or hitting her. The Director of Nursing Jeresel Resario is the WORST person for that job and needs to be fired! I am seeing help from the state with many complaints to ensure she gets held accountable for her actions and smart mouth she never responds to emails and was very RUDE (EYE ROLLING WHILE TALKING TO HER) AND NONCHALANT WHEN I TOLD HER MY MOTHER WAS PASSING AWAY!!! Lauren Williams the Unit Manager is also nonchalant. The receptionist lets the phones ring as if she screens their calls and the nurses' stations do not answer period! I currently have attorneys on my case and these people have been reported to the state for neglect and abuse! PLEASE PLEASE PLEASE DO NOT SEND YOUR LOVED ONE HERE OR ELSE YOU'LL BE BURYING THEM SOONER THAN EXPECTED AND GOING THROUGH A LOT OF STRESS OR READY TO FILE A LAWSUIT! read more

Do not leave your loved one in this place!!! Run for the hills they are unpleasant and uncaring people. It smells horrible and they cook food onsite that looks and taste terrible. I had to bring my Dad food in because he couldn't eat it. The smell overtakes the place. It is not sanitary. I complained and they gave me excuses about the patient being sick, but they should be cleaning at some point and deodorizing all they did was spray some air freshener and thought I would be satisfied. The ended up my Dad and he had to be rushed to the hospital where he had to have surgery he is already to weak and the unnecessary surgery could have done more harm to him because of his health conditions. You have to stay on top of the staff, and they will not keep them clean or shaved. Please do not use this Nursing facility for your family especially if they are not able to speak up for themselves. read more

Bradford Oaks Nursing and Rehabilitation Center located in Clinton MD, has to be the absolute worst place to send your loved one. My elderly father was placed there for rehabilitation last week. He was there for just four days. He had to be transported via ambulance to the Medstar Southern Maryland Hospital just across the street after a series of ongoing vomiting. His condition may have been caused by the awful "food" digested there. When I walked into his room two days ago, he was vomiting profusely all over his bed and the floor. After summoning the nurse three (yes I said three) times, I had no choice but to clean it up myself!! Finally a nurse walked in and told me she would just let the other nurses know that my father's food should be in the form of puree!!! This is ridiculous!!!! Understaffed is understated. I would NEVER recommend this place to anyone whom I care about: The food does not even have a decent appearance - what they called a "Sloppy Joe" looked like cut-up rubber bands with a side of ketchup!! Upon my arrival earlier this week, I saw an elderly female patient with a partial leg amputation who had rolled herself out of the front door. She was almost in the entry driveway. A relative arrived at the same time and proceeded to give the receptionist a piece of her mind..... Wow. The poor patients look so neglected. read more
