Eagle 1 - house If there were 10 stars they would get all of them. Please see pictures attached. As a clinician in the area, I found the Eagle 1 facility to be above reproach in all areas. I was honored to work with them as my cousin was on hospice. He lived at the Eagle 1 hours for approximately 2 years and passed away Nov 17,2024 and the staff did not leave his side. The management and staff went above and beyond what was required of them. He was treated daily with kindness and respect and will say he was quite spoiled. In his last hours staff came in and said their goodbyes. These are precious memories. They were there on their off time and would come in just to spend time. The Administrator and Supervisor both came in the night he passed away. They are more like family now and would not hesitate to place someone in their care. All Ashley Manors have only one caregiver per shift which is what corporate allows. At this particular location, where my father stayed for more than a year, he was routinely left soaking wet through his clothing and bedding. One time when I was there he had to wait 2 hours to go to the bathroom from the time he asked till the time the shift changed and one other person was there to help. The staff that I knew was remarkable in that they tried their absolute best but there is only so much one person can do for 12 residents in one building and 16 in the next building. When my dad left they were being audited by Idaho Department of Health and Welfare. The guy told me they have been researching complaints over and over again, but corporate won't budge on the decision to not allow an adequate number of employees per facility in order to see to the needs of the residents. Those needs are not being met. I had to go to the facility countless times to help out with my dad for that reason. I could never relax and trust he was getting adequate care. With the exception of a couple caregivers that work really hard, the entire experience at this facility was a frustrating, exhausting, and heartbreaking joke compared to what they promise in their sales pitch. No activities, they are all just in front of a tv all day long. Constantly understaffed, and the staff is shockingly unskilled and nowhere to be seen half the time, with just one person on the floor for the whole house. We visited my father as much as we could just to make sure he got basic care and assistance with eating and staying clean. I never witnessed much care being given to other residents when I was there for 5-6 hours at a time. They do not check them "every 2 hours" to stay dry. His roommate did not get along with him and didn't want to share space, so my father lived on the couch the entire 4.5 months. He lost 25+ lbs in just a few months, not just due to dementia, but the poor quality of food and lack of assistance, even when we provided snacks they were supposed to offer him. The admin and provider were never concerned with his weight loss or nutrition unless we pushed for it, and their philosophy is to heavily sedate any behaviors with no alternative plan for quality of life. Even at just over 100 lbs and no longer getting up on his own, they still had him on the same doses of several heavy psych meds by the time we switched him to hospice and moved him home. He passed soon after, a skeleton with terrible bleeding bedsores and rug burns on the backs of his ankles. Using heavy meds and not having skilled staff to prevent falls is inhumane. My father had a trip to the hospital from a fall due to negligence, as did others in the time we were there. We had no choice but to use the facility as a holding space for him during these months and it was an awful experience. I was never even given a care plan or communicated with AT ALL the first several months he lived there, with numerous falls and incidents occurring. I never once got a phone call reporting anything. It's absolutely criminal what they charge families for the lack of care and safety. read more

Ashley Manor - Rug burns from being pulled around on a rolling walker on the carpet - Eagle, Idaho

Never take your loved one to Ashley Manor in eagle Idaho. My grandma has advanced Alzheimer's. She has been at the facility for one month. In that time she has been to the ER 4 times, 3 for falls and once for an infection resulting from improper cleaning of a wound resulting from a fall. We went to transport her home today and she had no shoes and no jacket, it was raining. When we got back I went in to get her shoes and they were missing. They had been letting her walk around in her socks for days. One of the other patients had been walking around with urine in her diaper so long it had soaked through her pants, all three attendants were out back smoking. On top of that they double charged us for her first months care, billing both her account and my mother's. We are looking to move her ASAP. read more

Worse place ever to put anyone in cognitive decline and can't fend for themselves or ask for assistance. Serious communication issues with management, staff and provider. Had many falls which were not reported to the family. Always had mystery bruises and wounds that no one could give and explanation for. Hostile roommate which made it impossible for our dad to use his room. He basically lived on the couch in the living room for 5 months and we often found him just laying on the floor. We had no care plan the entire length of his stay. He was often left in wet and dirty clothes and never even got a shower the first 2 weeks he was there. They use heavy psychiatric drugs to minimize behaviors and all they did was sit like zombies in front of the TV. No other activities or therapies were ever offered as advertised. They are always understaffed. The food was no good and no alternatives ever offered. No assistance with eating or drinking. He was severely malnourished and suffered horribly from starvation.
