Having my husband at this rehab facility was a nightmare…read more After spending nearly two months out of state for back-to-back brain surgeries and serious complications, we were grateful to have him out of the hospital and closer to home. Unfortunately, after assuring us that they were equipped to care for his extensive medical needs, the care he received was deeply concerning. Some examples: -While in their care, my husband developed bilateral pulmonary embolisms. He was severely short of breath, yet it took nearly 24 hours for him to be sent to a hospital for treatment. Staff attributed his symptoms to COVID and dismissed repeated requests from our family to have him evaluated. When we asked if we could take him to the hospital ourselves, we were told, "You can't take him out those doors." -When they finally called an ambulance to take him to the hospital, they did not provide transfer information, including his medication list and other relevant medical details. -A nurse dropped an IV line on the floor, picked it up, and continued to use it.-Important medications, including pain medication needed to help him participate in PT/OT, were delayed by several hours. -Members of the nursing team blamed each other in front of us for improperly administering his IV antibiotic, which he was receiving for meningitis. -A bag of saline had a handwritten expiration date and continued to be used for two more days. When I asked the head nurse if it could be replaced, she appeared annoyed and said, "We'll replace it if it makes you feel better." -His PICC line was left so dirty that when he was eventually admitted to the hospital for the blood clots, the hospital nurses were afraid to use it because they did not want to risk another infection. We tried more than once to have open, honest conversations with the head nurses about our concerns, but we were met with defensiveness and dismissal. One morning, a nurse told me there was confusion about whether my husband had received his IV antibiotic because it had not been properly documented. When I asked for the head nurse to speak with her about it, she told me there was "no confusion," then turned around and walked out before I could respond. I would not recommend this facility to anyone. My husband was miserable there and said he felt like nobody cared about him. More than once, he was left yelling for help at night to use the bathroom, even though he could not go on his own. Based on our experience, I would not trust this facility with the care of a medically complex patient. We tried to approach these conversations with patience and understanding, but we were repeatedly made to feel like an annoyance rather than a family asking for basic communication and safe care. I also ask that the facility not respond by asking me to contact them to discuss this further. We tried repeatedly to speak with someone who would listen while my husband was there, when it mattered most.

I was left in my room for 6 hours the first day with no walker wheelchair or pain meds and was not evaluated until the next day to even use a walker. The aids never answered call buttons. On the second night there I had rung my call button for a glass of water and waited an hour and rang it again. I was answered by 3 male aids who told me other people needed them more and I should be nicer with raised voices and intimidating behavior. I called and left a message for the case manager that night and discharged myself the next morning. My records indicate I received care and evaluations that I did not receive. My records also indicate things that I had never said and my health records indicated many many mistakes about my medical history. It is a lovely rehab building but I would never recommend this for any one. The medical staff are incompetent to say the least.

What most people need to realize, is the importance of building a relationship with the staff caring for your loved one. It is a team effort. This is a care facility. They cannot and should not be expected to give private duty nursing services. It is unrealistic for any facility to be expected to do this. You have to be willing to reach out to the staff and work with them. Come to them with concerns, but also realize a thank you goes a long way when you do this very hard work. I can't begin to thank the staff for the care dad received especially during his final days. I stayed with my dad to care for him and make sure he knew he was not alone. I believe that was my honor and duty, but this staff cared for me as well as my dad. The concern for my well being touched me to my core. I will not forget the compassion my father and I received by all the staff. The flower arrangement that was sent to the funeral home was so beautiful and very appreciated. Staff who became friends were there at the funeral home to support me and my family. Their concern did not stop as dad was taken from the facility. If you are looking for a facility to "team care" for your loved one, stop looking, the Lakes of Sylvania should be strongly considered. I will only refer folks to this facility.

The communication which existed between Lakes of Sylvania and her family was also tremendous. Each day an updated Covid report was to be found in my email and a weekly video was posted to inform all families members of the events of the week. I was able the connect in person with the staff whenever I so desired. My dear one got the BEST of care and compassion from everyone within the walls of Lakes of Sylvania. Each house holds just 10 residents in 10 bedrooms, split between 2 hallways on either side of a central common area with dining space, living area, and an enormous kitchen where staff prepares meals and residents may use space, also. Each bedroom is private with en suite bathroom equipped with a walk in shower. Facility provides twin bed, sitting chair, side table, and dresser. There is space to personalize as well. We plan to add 2 sitting chairs for guests and a larger dresser. There is a large closet, between 5-6' wide. Laundry is done on a schedule and not mixed so chance if items missing is very low. Nurse is available, aids are assigned on shifts and are very capable and friendly. The center publishes activity calendar including social outings and shopping trips. There is a hair salon and manicures and pedicures can be scheduled.

There are only 3…read morenursing facilities in Oregon and the other two I have never heard a positive review about from anyone that has been in, or has had a loved one in. This one... I had never heard anything about. I mean zippo! So I did a Google search and found testimonials, 2 to be exact which should have been a big old red flag. And those two people with the forced smiles were in all the pictures with the staff too. Hmmm. But I really didn't have much of a choice, it was right down the street from my house (and my husband doesn't drive) and I was being booted out of the crap hospital. Now, I am not a complainer usually, but if you're not set up for wound care, then you shouldn't take patients that have such needs and that is what I was admitted for and the staff was clueless on how to wrap up my wounds. Practically had to do the shit myself half the time. Anywho... When I got to the home, number one I was guaranteed a private room by a rep from there and they wheel me into a room with a 94 year old woman that thought she deserved special treatment because her granddaughter worked there (more to come on that). They gave me a bucket of their toiletries that I had no use for because I still have my own teeth and some guy barked some order about getting me an air mattress,which I didn't get until I DID get my private room 4 days before I demanded to be discharged. That same night (actually about an hour later) I had to use the bathroom and found out at the same time that pressing the call button was a waste of time, so I had to clean up my roommate's puke all over the toilet seat. I knew right then that this place was going to be a fucking nightmare! They answered the call button about a half an hour later and I rudely told them that I didn't work there. I am also a smoker and they thought they were going to take my smokes and tell me that I can only have one every 4 hours with a babysitter. My ass! I wasn't there on a mental case and I will go out whenever I damn well please especially when I already had the codes to the doors. What were they going to do? Kick me out! Nooo! They only threatened the long stays with kicking them out. Putting them out on the street. How lovely. The food wasn't fit to feed a dog. When I got tired of getting cold food and eating bagged salad with about 4 hard boiled eggs on it, I asked to speak to the administrator and informed them that I had worked in dietary services at a hospital and a nursing home and that the Health Dept. would have a field day in there especially when the staff is walking around eating the food off of patient trays and they make sure the staff is fed before all the patients have what they need. Ask me how many times I went hungry there. The nurses also swap patient meds, I was offered a med from another patient when mine hadn't come in yet. Of course I refused and kept it in my memory to put on here. There was only one wound care nurse and she was great! And a guy in physical therapy that got me up walking and kept my spirits up and sanity in check. I will always remember them and be forever grateful for my speedy recovery. They certainly belong in a way better facility because that place and the rest of the staff should all be fired and replaced with people that actually give a damn.

Choice Senior Home Care specializes in long term care and getting consistent caregivers in the home that make your loved one feel safe and comfortable. Ultimately, my mom didn't need long term care but I would always consider going back to them if she ever does, and I would recommend them to my Michigan friends and family.
